NHCHW does not collect Personally Identifiable Information (PII) about you unless you choose to provide that information to us so that we can assist you. Some information we ask you to share is for statistical purposes only and will be reported to our funding source; Health Resource Services Administration (HRSA). None of the information that is reported contains any identifiable information including name, date of birth, or address. Any personal information you do chose to provide is protected by the security practices of HIPAA and 42 CFR Part 2 unless it is determined that, you, the user is a danger to yourself or others. Non-personal information related to our website may be automatically collected and temporarily stored. North Country Health Consortium does not disclose, give, sell, or transfer any personal information about our visitors unless required for law enforcement or by Federal law.

Media

If you upload images to the website, you should avoid uploading images with embedded location data (EXIF GPS) included. Visitors to the website can download and extract any location data from images on the website.

If you upload your resume or cover letter to the website, we will keep a copy on file for employment purposes.
