About This Event

The 2025 NH CHW Summit is a statewide gathering designed to empower, educate, and connect Community Health Workers (CHWs) and their allies. This year’s theme, “Building Bridges: Connecting Communities & Care”, highlights the critical role CHWs play in bridging gaps in healthcare and social services. Through engaging sessions, insightful discussions, and networking opportunities, attendees will gain valuable tools, resources, and inspiration to enhance their work and impact in their communities. Whether you’re a CHW, a healthcare professional, or a community advocate, this summit is a chance to learn, share experiences, and strengthen the CHW workforce across New Hampshire.
