CHWs-NH is excited to announce a new partnership with the New Hampshire Nurse Practitioner Association (NHNPA). Through this collaboration, CHWs-NH will have a prominent presence at the upcoming NHNPA Conference from April 3-5 at the Omni Mount Washington Resort. This is a valuable opportunity to raise awareness about the vital role CHWs play in improving health outcomes and reducing costs. By working closely with Nurse Practitioners, CHWs can enhance care coordination, advocate for patients’ needs, and help address social determinants of health (SDOH). This partnership aims to empower both CHWs and Nurse Practitioners to deliver comprehensive, patient-centered care for all individuals living in New Hampshire.

In addition to partnering with NHNPA, North Country Health Consortium is working with several other partners to implement the CHWs-NH initiative including NH CHW Coalition, Southern NH Area Health Education Center (AHEC), Community Workforce Institute, Bi-State Primary Care Association, Dartmouth Hitchcock, Regional Public Health organizations, and other stakeholders through a CHW Statewide Workforce group.
