Community Health Worker Summit 2024

Join us on Tuesday, April 30th to help LIGHT the future for the Community Health Worker workforce at the 2024 NH CHW Summit at the Grappone Conference Center in Concord, NH. This full-day event uplifts CHWs’ growing role improving access and outcomes.

Attendees will choose from educational breakout sessions, get important policy updates, help shape the NH CHW Coalition’s strategic direction, network with fellow CHWs and allies from across the state, visit vendor booths to discover new resources, and MORE!

We welcome all Community Health Workers (CHWs) and similar roles, employers, policymakers, healthcare and public health providers, program managers, and other supporters committed to investing in the power of CHWs to transform community health.
